Transitions at LifeWorks

What Is Transitions?

Transitions is a non-residential program facilitated by Program Director Linda T. Walsh, MFT, and designed to make the journey to adulthood easier for AS and NLD youth.  Transitions provides: case management, advocacy, social skills groups, life skills classes, work experience, and a safe gathering space for AS and NLD youth and wrap around support for their parents.

More Easily Navigate Disability Services

Case Management: The Transitions program director works closely with students and the Disability Resources Office of the local colleges to coordinate class selection, communicate with instructors, and acquire proper accommodations and tutoring assistance. Transitions students seeking paid employment are assisted in working with the California Department of Rehabilitation to secure and monitor employment opportunities.  Every Transitions student meets with the program director weekly to discuss their school/work progress and performance.  The program director makes clients accountable to someone other than their parents and ensures that they don’t get lost in the confusion of disability and educational services.

Advocacy:  Let Transitions advocate for you.  This may mean teaching our students the steps necessary to working with disability services or going to counseling meetings with Transitions students in order to help them to speak up. Advocacy services are available to ensure that our students are able to use the disability services system services in their progress and to give clients the necessary support to learn how to advocate for themselves.

 

Develop Friendships

Social Skills Groups: Humans have an innate need for connection to other people.  Although those with AS and NLD are known for not having close friends, this is not for lack of wanting or trying.  Transitions facilitators will provide: scheduled outings with other students to support friendships within the program, unscheduled social time on the premises, and problem solving groups for social issues that arise on campus or at work.

Safe Gathering Space: The Transitions center is open Monday through Friday. Transitions is housed in close proximity to the Santa Rosa Junior College.  This space is furnished in a comfortable, home-like manner, with kitchen facilities, computers, quiet spaces, and visiting areas. It is intended that this space serve as a drop in place for students between classes and at other times as well as the location for various program classes.  A staff facilitator will be present at all times the center is open to students. 

 

Gain Independence

Life Skills Classes: Ideally, Transitions students will become increasingly self sufficient. Transitions students learn basic independent living skills such as preparing a basic meal, paying bills, balancing a checkbook, looking for a roommate, buying a cell phone, calling customer service, going to the laundromat, and other necessary skills vital to successful independence. 

 

Internship Program: The Transitions internship program  collaborates with the California Department of Rehabilitation and local businesses to create work experience opportunities for Transitions students in school or for those hoping to seek and maintain employment.  The goal of the internship program is to give all Transitions students a practical and controlled experience in the work world. 

 

 

Learn How to Study Smarter

Tutoring: Transitions tutoring teaches students strategies to understand and strategize coursework.  Transitions tutors help students to perform at the best of their actual ability. 

 

Minimize The Stress of Change

Parent Group Schedule

Parent Support: Parent education classes and support groups will be provided so that Transitions students’ home lives are consistent with their Transitions work.  The Family Program includes: helping parents to trouble shoot specific problems, educating parents in how to teach and promote self-sufficiency, and to support parents in supporting their children.
